What a Support Specialist Does
-Develop rapport with youth, specifically youth that are difficult to engage to support a long-term permanency plan for the youth.
-Work in collaboration with the Family Engagement Specialists as part of the Permanency Team for identified youth.
-Conduct outreach to County and community partners.
-Keep well-written, concise case notes to include date, time and place of meetings for each participant, and any contacts made with other service providers and family members; entered in approved database.
-Collaborate with treatment teams, including County Partners at Child Welfare Services/Probation and other providers, keeping them informed of progress and challenges as necessary.
-Attend trainings related to Family Finding and apply findings from research on Family Finding best practices.
-Maintain professional behavior at all times, appropriately representing YMCA's mission and philosophy.
