Social Work Case Manager - TIDES

 Job Description        

•Provide outreach at schools and in the community and provide enhanced mental health case management and/or rehabilitative services to MHSA and Medi-Cal clients •Provide case management services designed to promote access to medical, social, rehabilitative, or other needed community services and supports for eligible individuals by providing consultation, coordination, referrals, and linkage •Provide mental health rehabilitative services to clients and families receiving outpatient mental health services •Facilitate psycho-educational and social skills counseling in a group setting •Maintain case records and other confidential materials •Maintain records to facilitate data collection and billing •Collaborate with school staff and other community based services to ensure high quality of care •Participate in training and supervision groups with supervisor and staff •Provide specialty care coordination for TIDES clients

 

 Qualifications        

•Minimum of a Bachelor’s Degree in Social Work, Psychology or related field required •One year of experience working with children and families in a mental health setting highly desirable •Bilingual (English/Spanish) required, multi-culturally competent individual required •Must be able to work during school hours, as well as, some evenings •Must be able to use computer and document in accordance with county standards/expectations •Ability to work independently and as part of a service team •ANASAZI data base knowledge desirable •Must have reliable transportation to travel throughout San Diego County •Visual and auditory ability to observe critical incidents and the physical ability to act swiftly in an emergency situation •Ability to adequately observe participants' activities, enforce safety regulations and apply appropriate policies and procedures •Must possess the ability to lead and interact in group activities and perform related physical skills
