 Job Description        

FRATERNITY HOUSE, INC., SEEKS AN EXPERIENCED HEALTH CARE PROFESSIONAL FOR THE ROLE OF RESIDENT CARE MANAGER. The Resident Care Manager is a key team member, responsible for carrying out the mission and purpose of Fraternity House, Inc. (FHI) as it relates to Directing & Supervising the care of Residents at its two licensed Residential Care Facilities for the Chronically Ill (RCF-CI); Fraternity House with eight (8) Residents and Michaelle House with ten (10) Residents. Under the direction of the Executive Director, the Resident Care Manager is responsible for all aspects of managing Resident care, including, but not limited to: Needs Assessments, Medicine Management and Case Management, managing and directing the care of the Residents, assuring Residents receive quality care at each house and are connected to services required for optimal outcomes, and creating and maintaining a comfortable, home-like environment for the Residents.



QUALIFICATIONS: 1. LVN, MA or CNA licensed in the State of California with at least three yearsí experience in the health care field, preferably in a licensed Residential setting. 2. Previous management experience and knowledge of management principles and practices required. 3. Knowledge of HIV/AIDS is required. Knowledge of Community Care Licensing regulations, alcohol and substance abuse, and mental health issues is preferred. 4. Sensitivity to working with culturally diverse populations. 5. Excellent verbal & written communication, interpersonal, and oral presentation skills 6. Computer proficiency a must. 7. Bilingual / Spanish a plus.


 Additional Information        

POSITION RESPONSIBILITIES: a. Review therapist evaluations, treatments objectives, and existing plans to determine Residentsí needs for therapeutic medical, psychosocial, and psychiatric evaluations. b. Assist to establish immediate and long-term therapeutic goals, to set Resident healthcare priorities, and help to develop a plan of care. c. Establish treatment programs by coordinating services & arranging resources. d. Enhance services available by identifying and facilitating services available through external agencies and community partners. e. Supervise and coordinate Resident care to ensure that services are carried out according to a physicianís order and the Residentís plan of care. f. Supervise caregiving staff in administration of Residentsí prescribed medications and treatments. g. Participate in bi-weekly on-call rotation for after-hours and weekend support. Cover caregiving shifts, as needed.
