Project Access Patient Care Manager

 Job Description        

All PASD patients receive care management services to ensure that patient’s healthcare needs are met efficiently, expeditiously, and compassionately. PASD care management services include: Patient care coordination: Coordinate referrals from clinics, determine eligibility. Identify barriers to care such as language and transportation, identify/arrange for access to these resources. Schedule appointments with specialists. Provide language interpretation at patient appointments. Schedule outpatient procedures and surgeries. Request donations of medicine, diagnostic equipment, DMEs. Data entry, billing reconciliation and reporting. Communicate Project Access updates to referring community clinics.



Bilingual in Spanish/English: written and oral communication. At least 2 years experience needed in health care delivery, case management, service coordination, or related health experience. Understanding of available community resources in San Diego County. Excellent interpersonal skills, including maintaining effective working relationships with patients, co-workers, community organizations and physician offices. Experience with managing competing priorities. An understanding of, and sensitivity to cultural and socioeconomic aspects of uninsured patient populations required in order to provide excellent service. Proficient knowledge of relevant computer software and data management systems, including Microsoft Office products and internet required. Meticulous attention to detail and excellent organizational skills are a must. Ability to work independently and problem solve on the spot, as well as be actively involved as a team member.
