 Job Description        

The Adolescent Family Life Program (AFLP) is a case management program for expectant and parenting youth age 21 and younger to: increase social and emotional support and build resiliency, improve pregnancy planning and spacing, increase education attainment and employability, and increase access to needed services. Case managers will be utilizing the evidence-informed Positive Youth Development (PYD) Model to improve the life course trajectory of expectant and parenting youth through a resiliency-based, youth-led program. What You Will Do: • Responsible for day to day supervision of (2) Case Managers and (1) Youth Advisor. • Provide managerial direction and support to staff in office and at off-site locations. • Represent the AFLP the agency AFLP program throughout the San Diego community and with contract resources. • Review case work of case managers and assist with difficult cases, ensuring quality care and effective interventions. • Attend program related meetings including case



• Master’s Degree in a Social Science, Public Health, or Health Education • Bilingual English/Spanish required • Previous management experience • 3 years of counseling/case management experience, in-home case management experience preferred • Previous experience working with the adolescent population • Comfort discussing sexual health information • Able to meet case documentation requirement regarding content and timeless. • Understands the importance of, and able to preserve, client confidentiality. • Knowledge of Early Childhood/Parent/Family Mental Health, Trauma Informed Care, Motivational Interviewing • Excellent written and oral communication skills • Sensitivity to cultural differences present in the organization’s/ program’s service population • Ability to apply good judgment and critical thinking to carry out instructions and make decisions within scope of authority
