Position Summary: Various duties to provide program oversight and staff supervision ensuring all services and programs are provided in alignment with the policies, procedures, and organization's values. Tracks established work plans for staff that include setting of clear goals and objectives and tracks staff performance in accordance with these goals. Provides monthly reports with information pertinent to program performance. Oversees program partnerships by maintaining relationships with contacts outside of the organization.In partnership with the Associate Executive Director, hires, trains, supervises and directs the work of the Care Coordination and Transportation Services Team. Generates staff trainings and leads team meetings and one-on-one supervision of each team member. Fills in as direct service provider when needed, which includes keeping a caseload of clients. Supports the administrative needs of food pantry’s daily operations.



The Care Coordination Manager oversees the Care Coordination and Seniors A Go Go programs. The Care Coordination Manager has a strong knowledge of home and community-based services for seniors as well as ElderHelp’s programs and services. This position ensures that all services and programs are provided in line with the policies, procedures, and values of the agency, and that the services provided are of the highest quality. The Care Coordination Manager will also carry a small caseload of clients, helping to increase our capacity and reach within the community. Bachelor’s Degree in Social Work, Psychology, Gerontology, or related field required. A minimum of 4 years of experience providing program management and oversight as well as supervising direct-service staff. A minimum of 5 years of experience working in the aging services sector. Candidate must have excellent communication and interpersonal skills, and be detail oriented. Bilingual (English/Spanish) preferred.


 Additional Information
