 Job Description        

Program Oversight •Develops and updates policies, procedures as well as program documentation as needed. •Tracks established work plans for program staff that include setting of clear goals and objectives and tracks staff performance. •Provides monthly reports to Associate Executive Director of client statistics, demographics, service utilization, impacts and outcomes, and other information pertinent to program performance. •Oversees program partnerships by maintaining relationships with contacts outside of the organization. Team Supervision •Participates in the hiring, training, and supervision of Program Team. •Motivates and develops a passionate, peer-driven team of direct-service professionals. •Plans and implements staff development activities. •Coordinates and attends regular team meetings. •Supervises each team member and conducts regular supervision to measure staff performance.



Bachelor’s Degree in Social Work, Psychology, Gerontology, or related field required. 2+ years experience providing program management and oversight as well as supervising direct-service staff. A 3+ years experience working with older adults. Candidate must have excellent communication and interpersonal skills, and be detail oriented. Bilingual (English/Spanish) preferred. Position Expectations: ElderHelp’s Program Manager is a knowledgeable, solution focused person. The Program Manager communicates effectively and actively listens while providing support to ElderHelp’s Program Team and assuring goals are achieved. The Program Manager oversees the Care Coordination and Seniors A Go Go programs. The Program Manager has a strong knowledge of home and community-based services for seniors.This position ensures that all services and programs are provided in line with the policies and procedures. The position carries a small caseload to help increase capacity and our community reach.


 Additional Information        

Other Functions: Care Coordination •Fills in as direct service provider when needed, which includes keeping a caseload of clients. •Completes comprehensive in-home assessment while visiting clients in their homes. •Provides trusted referrals to clients and their families as needed. •Maintains accurate and timely case notes, databases, and statistical reports. •Coordinates interventions with client’s support system and other agencies as needed. General: •Participates, as needed, in outreach activities and other events that help promote ElderHelp •Represents ElderHelp at meetings and other public events as directed by CEO/Executive Director and the Associate Executive Director. •Other duties as assigned.
