Case Manager / Transitional Facilitator

 Job Description        

*Support the Future planning process, including organizing team members *Coaches and provides technical assistance on employment and/or education * Facilitates roommate meetings and when needed, conflict resolution * Matches participants with a mentor when appropriate * Works with young adult to identify permanent connection * Maintains participant files * Provide resources as needed to permanent connection to facilitate their ability to support and connect with young person * Shares responsibility for 24-hour on call duty *Conducts case management meetings with family members, social workers and other support systems.

 

 Qualifications        

Bachelorís Degree in Social Work, Psychology, Counseling or related field; at least 24 years of age. Experience Requirements:* Minimum two years experience ideally in direct client services. Experience ideally includes working with at-risk adolescents and young adults. Additional skills preferred include: prior experience with case management; experience in the dynamics of human development; experience in the dynamics of child abuse, trauma, and foster care; an understanding of cultural issues; knowledge of and skills in assessment and intervention; knowledge of resources in the community; ability to collaborate with other agencies; and understanding of quality control issues. Must have a valid California driverís license and fingerprint and child abuse index clearances. Bilingual in English and Spanish preferred.
