WSS Case Manager

 Job Description        

This Case Manager position is responsible for managing an assigned caseload of adult patient having co-occurring needs, screening for potential patients, and following through with referrals, making and receiving follow up calls, documenting patient progress and working with other community resources in meeting/achieving patient treatment needs. This position is responsible for assisting with creating and implementing forms /documents using Microsoft office excel, word, etc. While working closely with the WSS Program Manager and WSS medical director The will assist providers and SUD counselors with managing case files; follow up services for each patient and tracking referral to the SUDS. While working directly with the patients, the Case Manager is required to have knowledge and understanding of, mental health, MAT, and substance abuse related issues. This position with be responsible for providing linkage to community services including services for severely mentally ill indivi



Must possess valid California driverís license & appropriate auto insurance as required by law Must have a working knowledge of substance abuse treatment operations, social services and community resources Education Required (Minimum level of education): Bachelor degree in social work or related degree (counseling, psychology) preferred but not required. Experience Required (Minimum Level of experience): Requires at least 2 years of working in the mental health and/or community outreach field, and office skills. Verbal and Written Skills to perform the job: Bilingual. Familiarity with basic word processing, spreadsheet, and database applications. Accurate keyboarding skills.
