 Job Description        

Position Type: $18-20/hour, Full-time (37.5 hours per week), non-exempt Benefits: Benefits for this position include a 401(k) plan, medical, dental and vision coverage, flexible spending accounts, life insurance and generous holiday benefits. Overview: The Health Homes Program provides care coordination services for Medi-Cal members who are diagnosed with certain chronic health conditions, have a high level of health care needs or are experience chronic homelessness. Working with the entire Health Homes team, the Program Assistant will provide administrative support in managing program data, verifying participant eligibility, preparing reports, and following up with program participants. The position requires a high level of comfort in working with multiple computer programs as well as a desire to work in a team environment. The position reports to the Director of Community Services. What You Will Do: • Manage and accurately input all program data from a variety of sources



You Will Need: • Bachelor of Arts preferred with exceptional computer experience • Fluency in English, bilingual and bicultural candidates (Spanish) are preferred • Comfort working in an environment that has multiple languages spoken and a diverse service population. • Sensitivity to cultural differences present in the organization’s/ program’s service population • Excellent computer skills, including knowledge of Microsoft Office, Adobe Suite and client management software. Prior data input experience using Microsoft Excel or Efforts to Outcomes preferred • At least one year of experience working in an office support role to a team of staff • Prior experience in the non-profit sector preferred • Excellent organizational skills • Ability to learn and utilize new computer applications • Ability to add, subtract, multiply, and divide all units of measure using whole numbers, common fractions and decimals • Ability to analyze data and deal with complex problems
