 Job Description        

The Care Manager is a full-time position working in the REACH2Caregivers Program and reporting to SCRC’s Clinical Supervisor. REACH2Caregivers program is an evidence based psychoeducational class series for caregivers of individuals with Alzheimer’s disease or related dementias. Classes are offered in group settings with other caregivers and focus on helping caregivers cope with their caregiving situation, take better care of themselves, learn stress management techniques and develop effective family communication skills. The Care Manager provides the REACH2Caregivers “early intervention” psychoeducational program as well as case management, care planning and consultation, and support services to caregivers and families of individuals with Alzheimer’s Disease or related dementias throughout San Diego County. The Care Manager facilitates a support group and works closely with the REACH Program’s Outreach Team on prevention outreach activities.



Master’s degree in a clinical discipline and/or health and aging discipline (MSW, MFT, MA, MSG). Clinical licensure or working towards clinical licensure preferred. Bilingual in Spanish/English required, including excellent verbal and written communication skills in Spanish and English. Ability to work effectively with interpreter services in the County’s threshold languages (e.g. Tagalog, Vietnamese or Arabic) as needed. Understanding of diverse cultures, health disparities and issues related to access of services. Knowledge and skills to thoroughly assess the needs of family caregivers including knowledge related to a variety of chronic and disabling health conditions, relevant community resources, helpful interventions, long-term care planning options, and issues related to caregiver mental health. Familiarity with health and mental health agencies, social service agencies and formal/informal support networks. Ability to maintain thorough and accurate case records

COMPENSATION AND BENEFITS Salary commensurate with experience. Full-time employees have access to excellent benefits including generous paid time off, sick time, and 11 paid holidays. Exceptional health, dental, vision, 403b retirement plan and mileage reimbursement. Position works 40 hours per week, typically Monday through Friday. Clinical Supervision: In-house supervision provided for individuals registered with the Board of Behavioral Sciences and in the process of licensure for LCSW or LMFT. Public Service Loan Forgiveness: Employment with SCRC, as a non-profit agency, allows individuals with direct student loans to apply for the Public Service Loan Forgiveness Program.
