Bilingual Crisis Case Manager and Services Navigator

 Job Description        

Qualifications To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Education and/or Experience: A bachelor's degree in psychology, social work or related field. Case management and client systems navigator experience with clients living with HIV/AIDS and LGBT clients strongly preferred. Two years' prior related experience also highly desirable. Additionally, experience working with AIDS Drug Assistance Program (ADAP) and Covered California enrollment experience is strongly preferred. Prior lived experience with population served is preferred. Language Skills: Fluency in English required. Fluency in Spanish required. Other Skills: Must demonstrate competence in working with a variety of diverse populations. Must have excellent interpersonal communication skills. Ability to manage multiple tasks with shifting priorities.

 

 Qualifications        

Position Summary The Bilingual Crisis Case Manager and Services Navigator is available at the agency for on-demand telephone and walk-in interventions. Duties include crisis intervention, triage, scheduling counseling intakes, medical and non-medical case management, and provision of information and referrals to community members and their families. Services are designed and provided within the context of a multidisciplinary professional team. It also includes completing ADAP and Covered California enrollments and VI-SPDAT assessments for San Diego's regional coordinated entry system for clients who are experiencing homelessness. The ideal candidate will be a self-motivated team-player who accepts direction, demonstrates excellent judgment, works hard, and is passionate about serving the LGBT and HIV communities.
