Case Manager Wraparound Services Program

 Job Description        

The Wraparound Services Case Manager will provide career guidance and counseling following the completion of a thorough individual assessment. -Collaboratively develops an individualized plan of care, implements this plan and evaluates its success over time -Provide intensive case management with a focus on assisting participants to overcome barriers and prepare them for employment that leads to self-sufficiency -Will require participant home visits or meet with participants at outside venues to provide case management services as needed -Maintain case management files ensuring confidentially; ensuring files are maintained as required by the company and contract -Develop and maintain ongoing resources and professional relationships within the community -Refer participants to appropriate agencies for specialized services and/or counseling -Provide and documents individual/group counseling and referral services -Utilize county and company computer data processing systems

 

 Qualifications        

-MSW Degree or current LCSW is required -At least 2-years of work experience as a Case Manager in social services, non-profit or for-profit organization -Working knowledge of the CalWORKs program and Welfare-to-Work component -Ability to organize, prioritize, and complete a large volume of work within strict time deadlines -Outstanding communication skills, both written and oral; must communicate professionally and positively with participants, staff, management, program partners, agencies, employers, landlords, mechanics, hotel/motel operators, and other service providers -A fundamental understanding of computer processing including word processing and spreadsheet applications -Maintain and establish effective working relationships with coworkers, agencies, employers, and other service providers -Ability to exercise appropriate judgment in releasing HIPAA/PHI information; analyze decisions and/or recommendations relating to the participant delivery of services
